چکیده

L s he role of plasma aldosterone concentration (PAC) in the athogenesis of hypertension took an interesting turn a few ears ago when several clinical trials claimed a higher revalence of primary hyperaldosteronism in patients hought to have primary or essential hypertension, particuarly those with resistant hypertension (1). A few clinical rials demonstrated that, on the basis of these data, the ddition of an aldosterone antagonist to the milieu of ntihypertensive medications led to significant reduction n blood pressure (BP) in patients with resistant hyperension (2). The debate has continued among experts in ypertension on the significance of PAC and the aldoterone/renin ratio (ARR) in the control and manageent of hypertension (3).

Aldosterone breakthrough does not alter central hemodynamics

INTRODUCTION Angiotensin-converting enzyme inhibitors and angiotensin receptor blockers are widely used in congestive heart failure and chronic kidney disease, but up to 40% of patients will experience aldosterone breakthrough, with aldosterone levels rising above pre-treatment levels after 6-12 months of renin-angiotensin-aldosterone system blockade. Aldosterone breakthrough has been associate...

Role of dopamine in the regulation of blood pressure and the renin--angiotensin--aldosterone system in conscious rats.

1. In conscious rats, intracerebroventricular injection of dopamine resulted in a decrease in blood pressure, whereas injection of metoclopramide, the dopamine antagonist, produced an increase in blood pressure. The central depressor effect of dopamine was attenuated by a subpressor pretreatment with intraventricular metoclopramide, but not by phentolamine. 2. Intravenous administration of dopa...

The spironolactone, amiloride, losartan, and thiazide (SALT) double-blind crossover trial in patients with low-renin hypertension and elevated aldosterone-renin ratio.

BACKGROUND There is continuing variation in diagnosis and estimated prevalence of primary hyperaldosteronism. The higher estimates encourage search for adrenal adenomas in patients with elevated ratios of plasma aldosterone to renin. However, it is more likely that patients with normal plasma K+ and aldosterone belong to the polygenic spectrum of low-renin hypertension rather than have the same...

Association Between a Polymorphism in the G Protein b3 Subunit Gene and Lower Renin and Elevated Diastolic Blood Pressure Levels

Gi proteins mediate the intracellular effects of many vasoactive and proliferative stimuli. Recently such signaling was found to be enhanced in cultured cells of some hypertensive subjects. A polymorphism at position 825 (C3T) of the G protein b3 subunit gene (GNB3) was strictly related to this phenotype. The aim of the present investigation was to test the association between this polymorphism...
